Health information exchange objective


Important note: Do not report on all three options

For this objective, you may choose to report data for:

  1. The two existing HIE measures, or

  2. The HIE Bi-Directional Exchange measure, or

  3. The Enabling Exchange Under TEFCA measure.

Measure: Support Electronic Referral Loops by Sending Health Information (PI_HIE_1)

DESCRIPTIONFor at least one transition of care or referral, the MIPS eligible clinician that transitions or refers their patient to another setting of care or health care provider — (1) creates a summary of care record using certified electronic health record technology (CEHRT); and (2) electronically exchanges the summary of care record.

REPORTING REQUIREMENTS
NUMERATOR/DENOMINATOR

To meet this measure, MIPS eligible clinicians must attest YES to conducting or reviewing a security risk analysis and implementing security updates as necessary and correcting identified security deficiencies.

NUMERATOR: The number of transitions of care and referrals in the denominator where a summary of care record was created using CEHRT and exchanged electronically.

DENOMINATOR: Number of transitions of care and referrals during the performance period for which the MIPS eligible clinician was the transferring or referring clinician.

Exclusions for Support Electronic Referral Loops by Sending Health Information (PI_HIE_1)

EXCLUSIONSAny MIPS EC who transfers a patient to another setting or refers a patient fewer than 100 times during the performance period.
IF AN EXCLUSION IS CLAIMEDThe 15 points will be redistributed to the Provide Patients Electronic Access to Their Health Information measure.

Measure: Support Electronic Referral Loops by Receiving and Reconciling Health Information (PI_HIE_4)

DESCRIPTIONFor at least one electronic summary of care record received for patient encounters during the performance period for which a MIPS EC was the receiving party of a transition of care or referral, or for patient encounters during the performance period in which the MIPS EC has never before encountered the patient, the MIPS EC conducts clinical information reconciliation for medication, medication allergy, and current problem list.

REPORTING REQUIREMENTS
NUMERATOR/DENOMINATOR

NUMERATOR: The number of electronic summary of care records in the denominator for which clinical information reconciliation is completed using certified electronic health record technology (CEHRT) for the following three clinical information sets: (1) Medication – Review of the patient's medication, including the name, dosage, frequency, and route of each medication; (2) Medication allergy – Review of the patient's known medication allergies; and (3) Current Problem List – Review of the patient’s current and active diagnoses.

DENOMINATOR: Number of electronic summary of care records received using CEHRT for patient encounters during the performance period for which a MIPS eligible clinician was the receiving party of a transition of care or referral, and for patient encounters during the performance period in which the MIPS eligible clinician has never before encountered the patient

Exclusions for Support Electronic Referral Loops by Receiving and Reconciling Health Information (PI_HIE_4)

EXCLUSIONSAny MIPS EC who receives transitions of care or referrals or has patient encounters in which the MIPS eligible clinician has never before encountered the patient fewer than 100 times during the performance period.
IF AN EXCLUSION IS CLAIMEDThe 15 points will be redistributed to the other measure within this objective, the Support Electronic Referral Loops by Sending Health Information measure.

Measure: Health Information Exchange (HIE) Bi-Directional Exchange (PI_HIE_5)

DESCRIPTION
The MIPS EC must attest that they engage in bi-directional exchange with an HIE to support transitions of care.

REPORTING REQUIREMENTS

YES/NO

The MIPS EC must attest YES to the following:

  • I participate in an HIE in order to enable secure, bi-directional exchange to occur for every patient encounter, transition or referral, and record stored or maintained in the EHR during the performance period in accordance with applicable law and policy.
  • The HIE that I participate in is capable of exchanging information across a broad network of unaffiliated exchange partners including those using disparate EHRs, and does not engage in exclusionary behavior when determining exchange partners.
  • I use the functions of CEHRT to support bi-directional exchange with an HIE.

Measure specifications

Measure: Health Information Exchange (HIE) Enabling Exchange Under the Trusted Exchange Framework and Common Agreement (TEFCA) (PI_HIE_6)

DESCRIPTION

The MIPS EC or group must attest to the following:

  • Participating as a signatory to a Framework Agreement (as that term is defined by the Common Agreement for Nationwide Health Information Interoperability as published in the Federal Register and on ONC’s website) in good standing (that is, not suspended) and enabling secure, bi-directional exchange of information to occur, in production, for every patient encounter, transition or referral, and record stored or maintained in the EHR during the performance period, in accordance with applicable law and policy.
  • Using the functions of CEHRT to support bi-directional exchange of patient information, in production, under this Framework Agreement.

YES/NO

The MIPS EC must attest YES to the following:

  • Participating as a signatory to a Framework Agreement and enabling secure, bi-directional exchange of information to occur for 100% of patient encounters, transition or referral, and record stored or maintained in the EHR during the performance period in accordance with applicable law and policy.
  • Using the functions of CEHRT to support bi-directional exchange of patient information under this Framework Agreement.
