Psoriasis: Improvement in Patient-Reported Itch Severity

DESCRIPTION:
The percentage of patients, aged 8 years and older, with a diagnosis of psoriasis where at an initial (index) visit have a patient reported itch severity assessment performed, score greater than or equal to 4, and who achieve a score reduction of 3 or more points at a follow up visit.

MEASURE PURPOSE:
This measure aims to improve pruritus in psoriasis patients in which symptoms are very burdensome; by assessing itch, aiming to make the symptom more manageable, and improving overall disease/response.

Q. Is this measure reportable via claims?

A. No.

Q. Are patient encounters conducted via telehealth allowable?

A. Yes. Encounters coded with GQ, GT, POS 02, or POS 10 modifiers are allowable.

A. The numerator are patients who achieve an assessment score that is reduced by 3 or more points from the initial (index) assessment score.

Q. What is the denominator?

A. The denominator are patients aged 8 years and older, with a diagnosis of psoriasis with an initial (index visit) NRS, VRS, or ItchyQuant assessment score of greater than or equal to 4 who are returning for a follow-up visit.

Calculation

The numerator: Patients who achieve an assessment score that is reduced by 3 or more points (minimal clinically important difference) from the initial (index) assessment score

The denominator: All patients aged 8 years and older, with a diagnosis of psoriasis with an initial (index visit) NRS, VRS, or ItchyQuant assessment score of greater than or equal to 4 who are returning for a follow-up visit.

Measure calculation example

Dr. Smith saw 100 adult patients return for a follow-up visit who have psoriasis during the reporting period. Of those, 89 patients had an initial visit where their itch assessment score was 4 or greater. Eighty-nine (89) is the denominator.

A total of 81 patients completed the itch severity assessment and of those 74 had a score reduction of 3 or more points. Seventy-four (74) is the numerator.

Quality of patient care = 74/89. Dr. Smith has a score of 83.1% for this measure. This translates to 9 to 9.9 points.

Important note

Because of how this measure is structured, physicians score 3 points even if they report on only one patient. This is applicable only to small practices with 15 providers or fewer. A score of 96.30% and above earns 10 points.
