Promoting interoperability (25%)


The Promoting Interoperability (PI) category focuses on interoperability and is worth 25% of the overall MIPS score. To participate, you must use a certified Electronic Health Record (EHR) listed at the Certified Health IT Product List (CHPL). You cannot satisfy the PI category requirements without having an EHR or getting an exemption from the PI category.

Points

There are 105 total points possible, but points are capped at 100. Each measure is worth a different amount of points based on how many eligible encounters you actually perform. 

You are required to report measures from each of the four objectives, unless you claim exclusions. To claim exclusions, click the “Measure Exclusion” checkbox during the PI submission process. Learn more about how the points for excluded measures will be reallocated by selecting from the PI objectives in the "PI objectives and scoring" section below.

For the Public Health and Clinical Data Exchange Objective, you must report on both the Immunization Registry Reporting measure and Electronic Case reporting measure, unless you claim exclusions. The other measures under this objective are optional.

EHR resources

The Academy provides comprehensive EHR resources, and our DataDerm registry supports PI reporting.

In addition to submitting measures, you must provide your EHR Certification ID from the Certified Health IT Product List and submit a “yes” to all of the following:

  • The Actions to Limit or Restrict Compatibility or Interoperability of CEHRT Attestation (previously named Prevention of Information Blocking)

  • The ONC Direct Review Attestation

  • The Security Risk Analysis Measure

  • The Safety Assurance Factors for EHR Resilience (SAFER) Guides Measure

  • ONC-Authorized Certification Bodies (ACB) Surveillance Attestation (optional)

Bonus points

There are 15 bonus points available between the measures that follow. You can earn 5 bonus points for reporting on any one of the following optional measures under the Public Health and Clinical Data Exchange objective:

  • Public Health Registry Reporting

  • Clinical Data Registry Reporting

  • Syndromic Surveillance Reporting

Note, reporting more than one of the optional measures under the Public Health and Clinical Data Exchange objective will not result in more than 5 bonus points.

Hardship exemptions

Small practices (15 providers or fewer) do not need to submit a PI Hardship Exception Application to request reweighting of this performance category, because CMS will apply an automatic exemption if no PI information is submitted. CMS no longer applies automatic reweighing for nurse practitioners, physician assistants, certified nurse anesthetists, and clinical nurse specialists, unless they are part of a small practice (no change from 2024).

Reporting period

Performance is measured for at least 180 consecutive days. To meet this requirement, the last day to begin participation is July 5, 2025.
